Case Management

The Targeted Case Management (TCM) Program provides Medicaid-eligible individuals aged 60 and over with comprehensive assessment, individualized care planning, periodic reassessment, and ongoing coordination and management of in-home and other supportive services. 

This program is specifically designed for individuals who are medically complex and at risk of, or in need of, a nursing facility level of care due to functional and/or cognitive limitations. 

A multidisciplinary care management team—comprised of professionals with extensive case management experience, including a Registered Nurse—uses a person-centered planning approach to develop a mutually agreed-upon service plan.

The goal is to help individuals maintain their independence, enhance autonomy, respect personal preferences, support caregivers, and promote the efficient use of available resources.
