Revised information- The Michigan Department of Health and Human Services (MDHHS) and the Office of the Inspector General (OIG) require agencies who receive Medicaid funding to actively detect and prevent Medicare and Medicaid Fraud, Waste, and Abuse (FWA). All reports of Fraud, Waste, and Abuse may be made anonymously. To report suspicions to the OIG directly: Call 855-MI-FRAUD (643-7283) (voicemail available for after hours) or submit an online complaint here or write to the OIG at Office of Inspector General, PO Box 30062, Lansing, MI  48909. You can also report suspicions of FWA to VAAA. Please report suspicions here.
