The Targeted Case Management Program (TCM) provides Medicaid eligible individuals with comprehensive assessment, plan of care development, periodic reassessment and ongoing coordination and management of in-home and other supportive services to individuals who are
aged 60 and over who are medically complex and at risk of, or in need of, a nursing facility level of care due to functional and/or cognitive limitations.
A multi-disciplinary care management team with comprehensive experience in case management, includes but not limited to, a Registered Nurse use a person-centered planning process to develop an agreed-upon service plan to assist the individual to maintain independence. The service is designed to enhance participant autonomy, respect participant preferences, support caregivers and promote efficient use of available resources.
